Acute Charcot Arthropathy: A learning experience
Abstract
Charcot Arthropathy, a rare condition, is difficult to diagnose and recognize early. We have maintained a prospective register at our hospital in Ashford & St peters Hospital since 2010. Until 2020 we collected 95 consecutive patients who presented with Acute Charcot. The management of Acute Charcot is not standardized nor scientifically proven even with an ever-increasing population of Diabetes globally not only it has a significant impact on the patient health it is one of the leading causes of amputation we have learned in our practice that by recognizing early and treating it in a scientific manner significant morbidity and mortality can be avoided. We have an MDT approach which involves vascular and Orthopaedics surgeons, Diabetes and Endocrine physicians, Interventional radiologists, Surgical Appliance team, Vascular scientists, Diabetes specialist nurses and Plaster Technicians. The format of this service is a one-stop clinic where patients with diabetic foot complications can be referred to by primary or secondary care professionals using a simple and clear referral pathway proforma to stratify the urgency of referral We have reported on the outcome and value of our MDT approach and the results are encouraging since the introduction of this service there has been a reduction in hospital admissions in diabetes related foot complications, an increase in limb salvage due to increased endovascular interventions, a rise in angioplasty and bypass operation and a decrease in the rate of minor amputation. In summary, since the establishment of our MDT diabetic foot clinic we have been able to identify diabetic foot complications at an early stage and intervene and optimize the diabetes and its complications. We are sharing our learning of recognizing the “Acute Charcot Syndrome”, the chronology of events and our treatment protocol. We also wish to share our outcomes in patients who have had Acute Charcot
